Teleheath Overview 

Teleheath provides closer monitoring of patients on a regular basis. The Telehealth Program was developed in conjunction with the Transitional Care Program to deter hospital readmissions.  The home monitor can be used to check:

• Blood Pressure 
• Blood Sugar
• Weight
• Heart Sounds
• Oxygen Level
• Pulse

All results are directly transmitted to your nurse at the local VNA office.
